Use this service to submit an NHS health check to the surgery.
The NHS health check tells us if you are at a higher risk of developing long term conditions, like diabetes.
What will happen at the appointment:
- appointment is for 15 minutes with a HCA or nurse
- we will ask some simple questions about your lifestyle and family history
- measure your height and weight (BMI)
- blood test
- blood pressure
NHS health checks are available every 5 years.
You can use this service if you:
- are registered at the surgery
- have been invited to do so
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
